A fantastic opportunity has been made available for a Scheme Coordinators position based in Stoke on Trent by my client.

This is a full time role for a period of three months with the view to extend.The rate of pay is GBP11.10 Per Hour PAYE Plus Holiday Pay OR GBP13.82 Umbrella/ LTD.

A current and enhanced DBS will be required for this role.

As a Scheme Co-ordinator, you will be responsible for carrying out an individual needs assessment and to develop a support plan with each customer which takes account of care and/or medical conditions and is particular to his/her own needs.

This will be to co-ordinate the delivery of appropriate support and care in accordance with these plans.

You will need to support the customer in all matters relating to their tenancy with the aim of maximising their independence and also meeting the terms of their tenancy agreement.

You will provide basic practical assistance such as form filling, resolving welfare benefit queries in collaboration with other key personnel, making contact or providing an advocate service with other agencies and care providers.

Part of your role will be to develop links with the statutory, voluntary and private agencies to ensure tenancies are sustainable and the Scheme Co-ordinator meets the needs, demands and aspirations of new customers.

You will be asked to provide written records and information relating to the action taken in the support of the customer.

In your role, you will be required to participate and contribute in support planning monitoring and case conferences with other agencies where appropriate.

An important aspect of your role will be to provide practical assistance with all tasks related to moving and setting up a new home, such as arranging removals, connection of amenities, liaison with appropriate agencies to acquire essential furniture and application for Community Care Grants/Loans and any relevant Social Services funding.

As a Scheme Co-ordinator, you will need to offer customers specialist advice and assistance in relation to claiming housing and welfare benefits, debts and other money matters.

You will be supervising the performance of contractors working within the scheme to minimise disruption to customers, protecting the safety of all on site and ensuring compliance with contractor specification, taking prompt action when shortfalls are identified.

Furthermore, you will be required to work closely with the Sheltered Housing Team Manager to facilitate a range of social activities in the scheme designed to actively promote the well being of customers and the local community.
