Our learning disability community teams provide outstanding generic and specialist care closer to home. We are seeking a motivated and enthusiastic learning disability nurse with significant experience of delivering and leading specialist community interventions as part of an Intensive Support model of care.

The Intensive Support Community team located in West Bromwich and covering Sandwell, Walsall, Dudley and Wolverhampton; provides a full range of assessments, interventions, support, training and advice to individuals themselves, their families and paid carers and the network of multi-agency stakeholders who are involved in the care and support of this complex client group.

Working within the Multi-disciplinary team, the successful candidate will provide a valued clinical leadership role. The post holder will work in conjunction with the Clinical Nurse Specialist, to develop and deliver a safe and effective service, in accordance with key national targets, clinical standards, service priorities and performance indicators set by local commissioners and Trust directive.

The post holder will work within the framework of an intensive support multi-disciplinary team to provide support and guidance to adults with learning disabilities who present with behaviours that challenge which place themselves or others at risk of serious harm; or for whom the nature or degree of risk might otherwise lead to exclusion, placement breakdown, and admission to inpatient services.

The post holders need to be caring, compassionate and committed to delivering high quality, meaningful and safe service in line with NMC, CQC and NICE standards.
