Settlement Support Worker
This is an exciting opportunity to make a differences to people lives, you will be working with young vulnerable adults age 16-25 and help shape their future.
Working closely with clients who have been offered accommodation you will be involved with their initial settlement period and duties will include:
Risk assessment of the clients needs
Support clients intensively for the first two weeks of their stay ensuring that they receive a full health check, register with a GP and access relevant health services identified at assessment
Ensure that all clients who are offered a service are moved into the service in a supportive, planned way and that all bedding, supplies and welcome packs are received by the client
Undertake a full induction with the client, including completing of the admissions checklist, housing benefit form, licence/tenancy agreement form and ensure that all aspects of health and safety are covered
Assist with completion of welfare benefits claims
Work alongside link workers to ensure clients are receiving the best service.
Advise and support clients during the moving on process, picking up from the Link worker, ensuring their move-on transition is smooth and managed.
This a hands on role and you will be required to cover multi sites, so you will be a full driving licence. This role will also require you to have a full enhanced DBS check.
You will be working on a set shift pattern, 8-4pm, 9-5pm, 2.30-10.30pm and may include some weekends.
