CARE CO ORDINATOR - FULL TIME

Responsible for Co-ordinating the Support Staff and Service Users, reporting all concerns, queries, and significant accident and or incidents to the Registered Manager

To maintain confidentiality on all aspects of care and your work within the company, and therefore respect the Data Protection Act 1998

To ensure that all people employed or using the service are treated fairly under the Equality Act 2010 and all it encompasses e.g. disability discrimination, equal opportunities etc

To have a working knowledge and adhere to policies and procedures and health and safety regulations in respect to your position.

Assist with the recruitment, interview and selection of new staff members

Ensure that new employees successfully complete their induction programme under the supervision of yourself, and other senior staff members.

Develop and instigate appropriate training plans for each new member of staff, and make sure they complete all appropriate trainings, including NVQ training.

Provide continual supervision and position monitoring to support workers throughout the organisation

Carry out monitoring and checks procedures and progress notes to ensure the appropriate care and support are provided to each individual

Assist with the induction of new service user referrals, assessing people’s needs and desires and develop a plan to provide the care and support and thereafter, monitor and review each person’s service

Deputise for your manager in his or her absence

Participate in investigations and staff disciplinary procedures

Manage on-call responsibilities as required, and be able to respond to the needs of service users as they may arise

To support senior members of staff in all aspects of the company’s general day to day running and management

To be a valued member of a team and to contribute to the overall development of the organisation that provides quality support services and promotes innovative approaches

Ensuring that all complaints are recorded and dealt with according to the company’s complaint procedure

Assist the Registered Manager to ensure all service users receive an individualised and fitting support plan/service that enables them to live their lives as independently as possible and in accordance with their needs, dreams, and goals. Ensuring that support plans are ‘live’ at all times

Ensuring risk assessments are in place where necessary and are reviewed regularly

Undertake as defined by the Registered Manager processes to monitor the quality and appropriateness of the service e.g. house visits

Have regular contact and build good relationships with the service users, liaise where appropriate with family and other carers

Ensuring regular service users review occurs and attending reviews when required

Undertake specified training and courses at the discretion of the management.

Provide regular feedback to the Registered Manager

Be competent and confident in using Microsoft Office package

Attend weekly office meetings with the Registered Manager and other Coordinators

Hours need to be worked flexibly to meet the needs of the service

SUPPORT STAFF

* Assist the Registered Manager in the Induction of new staff. i.e. completing ‘Generic Shadow duty form’

* Responsible for passing on information regarding staff performance for supervisions purpose

* Arrange cover for staff absences (when required)

* Ensure regular contact with the staff team and provide them with support as required

* Encourage an open, supportive, and positive team climate.

RECRUITMENT

* Identify and notify the Registered Manager of any need to recruit staff

* Assist the Registered Manager with the selection and recruitment of new staff when required

TRAINING

* Ensure that any urgent training needs identified are immediately relayed to the Registered Manager

* Comply with all training requirements expected to enable duties specific to their duties are fulfilled

LEGISLATION & COMPLIANCE

* Ensure all paperwork generated by the service is completed, accurate, signed where applicable

* Ensure a good knowledge of Policies & Procedures and the Staff Employment Guide

* Ensure all Support staff have read and understood their Staff Employment Guide

* Ensure all complaints or concerns are appropriately reported and dealt with

* Report all instances of abuse or suspected case of abuse immediately to the Registered Manager

* Liaise with Social services and Supporting people where appropriate
