Mental Health Transition Worker

Birmingham

6 month FTC

£22,000 to £24,000 per annum, dependent on experience

CMD Recruitment are delighted to be working with this charity, who engage with people experiencing mental health problems, to recruit a Mental Health Transition Worker on a 6 month fixed term contract.

About the role

As a Mental Health Transition Worker, you will develop and support pathways for transition and progression into a range of opportunities for daily activities and social support. You will work as part of a team offering information, advice and guidance to adults who have mental ill health.

You will effectively engage a group of adults who have mental ill health to enable them to explore their available opportunities for leisure and community activities.

General Responsibilities

  • To take responsibility for meeting the requirements of all health and safety legislation
  • To implement the policies and procedures of the organisation, including health and safety procedures, complaints and serious incidents
  • To work according to the equality and diversity policy of the organisation
  • To ensure the efficient use of resources at all times
  • To respect confidentiality at all times
  • To participate in staff meetings, supervision, training and personal development and any other meetings as required
  • To undertake any other duties appropriate to the nature of the job

Essential Skills, Knowledge and Experience

  • Possession of, or working towards, an NVQ Level 3 in Advice & Guidance
  • Ability to travel across the city
  • Knowledge of barriers faced by people who have mental ill health
  • Well-developed interpersonal skills including an ability to communicate with people at all levels
  • Ability to maintain accurate case records using a database and written client file records
  • Ability to organise own workload and meet deadlines
