An exciting opportunity has arisen within an Intermediate Care service in Shrewsbury for an excellent Social Worker with relevant experience.

This service works as a part of a Multi-Disciplinary Team focusing on 'home first' to maximise independence. The team ensures the provision of short-term support at home following discharge to avoid unnecessary re-admission to hospital.

As an Adults Social Worker working in this team, day to day you will be responsible for providing multi-disciplinary rehabilitation to support people in regaining and maintaining their independence, assessing, setting goals and developing a rehabilitation plan. You will be working with complex and challenging cases with full access to support and supervision.

To apply for this role you must hold a degree, post graduate or Professional Qualification in Social Work with full HCPC registration. Ideally you'll have 2 years post graduate experience in Adults Social Work, have Clear and valid DBS disclosure, and have access to your own car.
